Our Clinical Pharmacist will join our experienced PCN team. You will be mentored and supported by a friendly and experienced clinical team, and will thrive on maximizing your potential.
Job ResponsibilitiesThe post holder will work within their clinical competencies to provide specialist pharmaceutical support to all practices of the PCN, including working with members of the wider healthcare team to improve care and support safe and effective prescribing.
The role involves patient facing activities either face to face or by telephone, including chronic disease management, repeat prescription management, and structured medication reviews. The post holder will also manage and review patients with polypharmacy.
The post holder is expected to be proactively involved in quality improvement and audit, as well as managing some aspects of the Quality and Outcomes Framework, and to contribute to provision of out of hours and extended hours/improved access services. They will be supported to help develop the role and work to improve care and outcomes for patients across South 1 PCN, playing a pivotal role in the delivery of high quality primary health care.
Person Specification Qualifications- 12 months experience working in General Practice
