Part-Time Social Prescribing Link Worker - Youth & Families

Cranstoun's Children and Young People's Social Prescribing Service in Dudley invites applications for a Social Prescribing Link Worker to support young people aged 11-25 and their families. The role covers Dudley and Sandwell as a pilot family social prescribing project.

You will provide flexible, person-centred support, manage a caseload, and collaborate with NHS, VCSE, and community partners to improve wellbeing and reduce isolation. DBS disclosure required.
