Part-Time Social Prescriber for Community Wellbeing

West Suffolk Clinical Commissioning Group is seeking a part-time Social Prescriber to join the Pro-Active Care and Practice teams at Ardleigh Surgery and Riverside Health Centre. The role is fixed-term until 31 March 2027, 21 hours per week, supporting patients with holistic, person-centred care and community referrals.

You will work across multidisciplinary teams, build links with community groups, and help reduce unnecessary GP visits through tailored support and behavioural change approaches.
