Part-Time Community Health Family Practitioner

HCRG Care Group in Lancaster is seeking a Health Family Practitioner to support the Lancashire Young People & Healthy Families Service. The role delivers mandated health reviews and family-centred care across community settings, including home visits, with a focus on child development and family health.
