A leading healthcare provider in the UK is seeking a part-time Clinical Pharmacist to join their team at Portland Medical Centre. The successful candidate will manage chronic disease reviews and support GP staff with medication queries. Ideal applicants will have prescribing qualifications, experience in chronic care management, and excellent communication skills.
