The Nurse Navigator functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for patients. The role involves ensuring all patients receive quality and comprehensive services, coordinating patient care throughout the continuum in collaboration with the multidisciplinary team. The Nurse Navigator serves as a clinical resource with expertise and acts as a liaison within the facility and in the community. Responsibilities include providing expert nursing care, direct clinical practice, consultation, and education. The role also involves participating in initiatives to improve health outcomes and health equity.
Duties and Responsibilities:
- Provide nursing care and guidance to patients, assessing, planning, implementing, and evaluating according to the nursing process.
- Educate patients and families, ensuring seamless, continuous, and comprehensive care. Document patient teaching based on assessed needs, including psychosocial and lifestyle factors.
- Assist patients and care team members with navigating chronic diseases, treatments, outcomes, and care plans.
- Assess educational needs of patients, families, and caregivers.
- Educate about diagnosis, treatment options, side effects, care team roles, and treatment adherence.
- Identify barriers to treatment goals, adherence, and appointment compliance.
- Address medication adherence issues.
- Facilitate scheduling of appointments, diagnostics, and procedures with the appropriate team.
- Identify overdue visits, labs, and referrals; contact patients and arrange follow-up.
- Assist patients in accessing healthcare and understanding health behaviors.
- Connect patients with community resources and support services.
- Facilitate communication among the multidisciplinary care team.
- Respond to patient inquiries about their care and community resources, documenting appropriately.
- Work in an organized, timely manner, partnering with patients, families, and the healthcare team for coordinated care.
- Review and update policies related to services provided.
- Participate in community educational events and staff education.
- Protect patient confidentiality.
- Perform other duties as assigned.
Requirements:
- Bachelor of Science in Nursing required with at least 5 years of nursing experience.
- Nurse Navigator experience preferred.
- Current RN licensure in Rhode Island and CPR certification required.
- Nurse Navigation Certification preferred and/or obtained within 6 months of hire.
