We are looking for a dedicated Mental Health Practitioner to join our Primary Care Network (PCN), working across 4 x GP surgeries. This role offers the opportunity to provide timely mental health support to patients within primary care, working closely with GPs, nurses, and other healthcare professionals.
You will deliver assessments, brief interventions, and care planning, while helping to connect patients with specialist mental health services when needed. This is a key role in improving access to mental health care and supporting the wellbeing of our local community.
Main duties of the jobAs a Mental Health Practitioner within the PCN, you will -:
Provide timely mental health assessments, advice, and interventions in primary care.
Support patients presenting with common mental health conditions (e.g., anxiety, depression, stress).
Work with GPs and practice teams to develop and review care plans.
Identify safeguarding and risk concerns and escalate appropriately.
Liaise with secondary mental health services and community providers to ensure smooth referral pathways.
Contribute to health promotion and wellbeing initiatives across the PCN.

Job responsibilitiesConduct mental health assessments and provide brief interventions.
Support patients with common mental health conditions, such as anxiety and depression.
Collaborate with GPs, nurses, and other professionals to develop and review care plans.
Identify risk and safeguarding concerns and escalate appropriately.
Liaise with community and secondary mental health services to ensure effective referral pathways.
Person Specification Experience- Registered and qualified mental health professional (e.g., RMN, Social Worker, Occupational Therapist).
- Experience working with patients with mental health needs.
- Knowledge of common mental health conditions and interventions.
- Ability to assess, plan, and manage patient care safely.
- Strong communication and teamwork skills.
- Experience in primary care or community mental health.
- Knowledge of local mental health services and referral pathways.
- Experience working across multiple GP practices or multidisciplinary teams.
