Job Description
Mental Health Nurse Practitioner, Hope Service (B6-7 Development Role)

The closing date is 27 April 2026.

This is a development role within the Hope Service for young people aged with complex mental health, behavioural and social care needs. The successful applicant will be a Band 6 or Band 7 Mental Health Nurse Practitioner who works with a multidisciplinary team to provide assessments, risk management, intervention planning and support for the young people and their care networks.

Job responsibilities
  • Work closely with the Mindworks alliance and crisis teams during office hours to respond to crisis and urgent referrals of CYP and manage their care from assessment to discharge.
  • Conduct autonomous clinical assessments of CYP with complex co morbid mental health needs, and establish risk management and treatment plans.
  • Ensure timely risk assessment and management in partnership with Emergency Departments, Crisis Intervention Service, Extended Hope, CCISS and other services.
  • Manage a caseload from referral to discharge, maintaining accurate documentation and timely completion of required outcome measures.
  • Contribute to audits, research and evidence based practice to improve the service's effectiveness.
  • Interface with other services and agencies regarding care, treatment and discharge planning.

Band 7 Key Responsibilities (in addition to those listed for Band 6):

  • Lead and supervise junior team members and contribute to their appraisal process.
  • Provide formal reports and advice to referrers and agencies on assessment and treatment outcomes.
  • Represent the Hope Service at professional and network meetings to advise on complex cases in crisis.
  • Collaborate on developing systems to monitor and evaluate the service.
  • Make independent decisions regarding risks and needs of young people and suggest appropriate interventions.
  • Undertake relevant CPD to maintain and improve assessment and treatment skills.
  • Act as deputy for the Clinical Lead Nurse when required.
Person Specification Education
  • Post graduate level qualification in Nursing with a focus on mental health or child nursing.
  • Current NMC registration.
Experience
  • Experience working in child and adolescent mental health, preferably in crisis pathway provisions.
  • Band 7 level experience of working with CYP with complex mental health needs.

Salary

£40,617 to £56,908 a year, pro rata, including HCAS.
