Mental Health Crisis Discharge Liaison

BRISTOL MISSING LINK is seeking a Female Facilitated Discharge Worker to join the North Bristol Crisis Team, based at Gloucester House, Southmead Hospital. This is a permanent, full-time role (37.5 hours/week) Monday to Friday 9am-5pm with some unsocial hours.

Salary starts at £26,577 per annum plus benefits. You will act as a link between inpatient wards and community teams to support safe discharges, assess barriers, and help reduce admission lengths.
