Medical Social Worker

On behalf of our client in public health, AllPro Recruitment are now seeking a Medical Social Worker to support an integrated care service for older persons in Dublin.

The successful candidate will work as part of a multidisciplinary team, supporting older people and their families with the social, emotional and practical challenges associated with ageing, illness and changing care needs.

Key Responsibilities

• Carry out comprehensive social work assessments and contribute to individual care plans.
• Provide support, guidance and advocacy to older people and their families or carers.
• Work closely with healthcare professionals and community services to coordinate appropriate supports.
• Support care planning and transitions between hospital, home and community services.
• Identify safeguarding concerns and respond in line with relevant policies and procedures.
• Promote each person's independence, dignity, wishes and involvement in decisions about their care.
• Maintain accurate, confidential records and contribute to multidisciplinary meetings.

Requirements

• A recognised qualification in Social Work.
• Current registration with CORU as a Social Worker.
• Strong assessment, communication and interpersonal skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Good organisational skills and the ability to manage a varied caseload.
• Experience working with older persons, in medical social work or in a community healthcare setting would be an advantage.

Salary in line with HSE pay scales, subject to recognised experience and approved scale placement.
