Job Purpose

As the first point of contact for patients, the Care Navigator receives, assists, and directs patients to the appropriate service or healthcare professional in a courteous, efficient, and effective manner. The role also supports the practice team and promotes a positive, friendly image for patients and other visitors, whether in person or via telephone.

Key Duties & Responsibilities
  • Administration: Maintain thorough knowledge of all practice procedures; work in accordance with written protocols; pull, file, and update notes; fax and photocopy as requested.
  • Reception: Receive patients, consult with members of the practice team, and cover all reception positions as necessary.
  • Appointments: Handle telephone calls, book appointments, manage queries, forward messages, deal with visits/requests, and communicate with patients face to face.
  • Computer: Register temporary and IM patients; enter data into computer systems and manage medical records; process patient address changes and update records.
  • Telephone: Operate the telephone system during and after hours; cover for the telephonist when required.
  • Other Tasks: Ensure building security, including knowledge of doors, windows, and alarms; perform additional tasks allocated by managers; maintain a caring environment with colleagues.

  • Health & Safety: Comply with the Health and Safety at Work etc. Act 1974 and take responsibility for personal safety and that of others.

  • Communication: Effectively communicate with team members, patients, and carers; recognise alternative communication needs and respond accordingly.
  • Safeguarding: Follow practice safeguarding processes and procedures; report concerns confidentially.
  • Training & Performance: Participate in training programmes and annual performance reviews; maintain a record of professional development.
  • General: Undertake other duties within competence; work across AMP sites; adapt to changes in opening hours, including weekend work; be flexible and accommodating.
Person Specification
  • Qualifications: GCSE grade A to C in English and Maths.
  • Experience: Experience working as a Medical Receptionist.
  • Knowledge: Experience with EMIS Web, Docman, and Klinik systems.

This role requires a Disclosure and Barring Service (DBS) check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
