Job Description

Greensand Surgery are looking to welcome a pro-active LTC practice nurse to join the team on a part-time basis for 16 hours per week. The successful candidate will provide and maintain a high standard of chronic disease management, which includes diagnosis, monitoring and development of individual management plans and agreeing these as appropriate with the patient and other health professionals. You will be managing a range of specialities including heart failure, diabetes, CHD, and CVD.

Main duties of the job
  • Be proactive in chronic disease specifically CVD, HF, hypertension, asthma and diabetes, QoF and enhanced service management
  • Responsible for management and running of long-term condition clinics in diabetes, chronic heart disease and heart failure in line with NICE guidance and Quality and Outcome Framework
  • Recognise signs and symptoms that may indicate undiagnosed long term conditions and utilise relevant referral pathways
  • Utilise Ardens templates to complete thorough and concise annual chronic disease reviews
  • Work with other clinicians and staff to provide education, support, specialist advice and guidance to colleagues
  • Refer patients to other services/agencies and members of multidisciplinary teams as necessary
  • Complete medical reviews
  • Proactive in improving and influencing patient care pathway
  • Attend and participate in nurse meetings
Qualifications
  • Current NMC registration with an active PIN.
All applicants should have the following qualifications, registrations, skills and experience:
  • Experience in primary or community nursing care
  • Accredited diploma (or above) level award in at least one chronic disease e.g. diabetes, hypertension, CHD or HF
  • Experience of continued professional development
  • Minor Illness experience
  • Experience of SystmOne and using Ardens templates
  • Asthma and COPD management
