Leaving Care Personal Advisor - Pathway & Independence

Nottingham City Council is seeking a Personal Advisor - Leaving Care Team to provide high-quality, person-centred support to young people leaving care and assist their transition into independent adulthood.

The role involves managing a caseload of Children in Care and Care Leavers, developing and reviewing personalised Pathway Plans, and offering practical, emotional and developmental support to foster independence, resilience and long-term stability.
