Lead Social Worker, Adult Physical & Sensory Team

Health Service Executive in Ireland is seeking a Social Worker to lead the Adult Physical and Sensory Team 4626CHCM across North Dublin, Meath, Louth, Cavan and Monaghan. This permanent, full time post supports people with physical and sensory disabilities and requires coordinating multidisciplinary teams to deliver person centred services in a complex care environment.

The successful candidate will work within the Disability Services Directorate, engage with families and partner agencies, and
