NHS GM Bolton locality Funded Care Team has an exciting acting up development opportunity for a motivated nurse to join their NHS Funded Care Team as a Lead Nurse Assessor.

You will be a registered nurse with experience and interest in community nursing or managing patients with complex needs. You will have experience and skills in case managing patients and carrying out assessments in a variety of establishments and in managing a clinical team. You will be highly motivated, have the ability to work independently, but also as part of a team. You will have good interpersonal skills and give attention to detail.

Main duties of the job

The redesign and growth within Bolton's NHS Funded Care Team has some exciting opportunities to join a committed and dedicated team of nurses. This is an excellent and exciting opportunity for developing Registered Nurses to join a committed NHS Funded Care Team as a Lead Nurse Assessor at Band 7.

The posts will require case management of complex cases, the completion of detailed health needs assessments and reviews in line with current legislation and local policy regarding Continuing Health Care. There will be extensive liaison with clients, families, care providers and other professionals within the health and social care sectors. The post holder will also be required review retrospective cases preparing assessments to be presented at funding panel.

The successful candidates will need to be able to demonstrate the ability to complete comprehensive nursing assessments, have good written and verbal communication skills, must have the ability to work in stressful situations and be able to priorities and organise their own workloads to meet deadlines.

Job responsibilities

The post holder will develop working relations with case managers, health professionals, and social care providers to ensure that NHS Greater Manchester Integrated Care Bolton (hereafter referred to as NHS GM NHS GM INTEGRATED CARE Bolton) is able to deliver appropriate services for people who are eligible for Continuing Healthcare and joint funding packages.

To work closely with external agencies including care in the community, provider agencies, and stakeholders in the design, development and commissioning of community services for identified individuals.

To work closely with Childrens Service Teams, to obtain evidence that will enable eligibility for Continuing Healthcare to be established and appropriate placements for patients to be made for transition.

To develop costed service design solutions, in conjunction with local partners, for Continuing Healthcare clients who are eligible for joint funding.

Qualifications
  • Registered nurse, Level 1 or equivalent
  • Masters Degree or equivalent experience
  • Registered Mentor
  • Advanced Knowledge of Continuing Health Care
Skills Knowledge and Experience
  • Proven experience in adult assessments
  • Experience of leading teams, managing performance and workload allocation.
  • Broad range of clinical experience including case management
  • In depth theoretical and practical understanding of continuing care criteria and issues
  • Experience of working within a multidisciplinary environment, and with health care professionals.
  • Specialist knowledge within CHC and managerial knowledge, combination of courses towards level 7
  • Experience in working within a multidisciplinary decision making Panel
  • Advanced Knowledge of Continuing Health Care
  • Experience in undertaking holistic assessment

  • Registered with the NMC
