Job Description

Hammersmith & Fulham Council is seeking a Family Support Practitioner to join its Early Help service. You will manage your own caseload, complete Early Help Assessments and coordinate Team Around the Family (TAF) meetings to support children, young people and families.

Direct work will take place in family homes, children's centres and community settings. You will work collaboratively with schools, CAMHS and health services to build lasting change while safeguarding and promoting welfare.
