Impactful Community Assessment & Care Planning Officer

Kirklees Council is seeking a Community Assessment Support Officer to help people live independently by delivering personalised social care assessments and reviews. You will work with customers, carers and health professionals to identify needs and develop person-centred plans.

You will join a supportive team committed to professional development and delivering high-quality, individualised care. A full driving licence is essential as travel is a key part of the role.
