Waythrough launched in October 2024 following the merger of Humankind and Richmond Fellowship. We tackle poverty and disadvantage in communities through mental health, drug and alcohol, housing, and related support.
We have almost 200 services across England and nearly 3,500 staff and volunteers who run them. Annually, our services support around 125,000 people.
Bringing together two large organizations is complex and will take time to fully integrate our systems, processes, and approaches. As a result, some jobs are still branded under 'Humankind'. Next year, we will fully integrate our recruitment systems, but for now, they remain separate.

Contract: Fixed Term (Until 30/06/2026) - Full Time
We are excited to offer a Hospital Liaison Worker role, working between the Together recovery service and local hospitals.
About the roleThe Hospital Liaison Worker role involves working between the Together recovery service and hospitals. Using a caring, compassionate, and trauma-informed approach, you will engage with individuals admitted to hospital with substance use support needs. You will deliver interventions such as advice, harm reduction, and psycho-social support, working closely with hospital colleagues to ensure appropriate support is provided.
This role involves establishing a high-quality hospital liaison service within the Together integrated alcohol and drug service, including assessments, interventions, and co-developing discharge plans to reduce relapse and readmission. You will communicate effectively with hospital departments (e.g., Mental Health, Hepatology, A&E, Maternity) and external community services (e.g., GPs, outreach, housing, community justice).
You will also provide training and guidance to healthcare professionals in managing alcohol and drug-related issues, including delivering brief interventions.
Responsibilities- Attend hospital ward rounds and maintain communication with hospital staff.
- Respond to hospital liaison referrals.
- Deliver interventions face-to-face and via phone.
- Manage a caseload, providing psychosocial interventions.
- Work within a multidisciplinary team for assessments, risk management, and community referrals.
We seek an empathic, caring, and organized individual passionate about trauma-informed and recovery-focused work. Transferable skills are welcome, with training provided. You should have:
- Knowledge of substance misuse and recovery.
- Experience in workload management and delivering high-standard work.
- Understanding of safeguarding.
- Excellent communication, organization, and time management skills.
- Ability to work well in a team environment.
