Hospital-at-Home Mental Health Practitioner

A healthcare provider in Fareham is seeking a dedicated Mental Health Practitioner to deliver compassionate, person-centred care. You will provide intensive home-based treatment, support patients and their families at critical points in their care journey, and work as a lead practitioner managing a flexible caseload. The ideal candidate will be a registered mental health professional with strong skills in assessment and clinical decision-making, as well as a commitment to recovery-oriented care.
