Oak House provides rehabilitation and recovery services for adults with complex and enduring mental health difficulties. Oak House has a supportive and motivated multi-disciplinary team that takes pride in ensuring that patients have a positive experience during their admission. Being part of our experienced team would allow you the opportunity to develop a broad range of skills to assess and support patients to develop or rebuild their skills and confidence in activities of daily living and accessing community activities. We will provide you with training and resources to develop a long term career, continual professional development and a range of support. You will be required to work 18.75hrs per week which could include a mixture of short days and 11.5 hr shifts to include working days, nights, weekends and bank holidays. Please note the Trust will not routinely provide sponsorship to work for this role. In addition to meeting the Home Office salary threshold you must have the significant experience to be offered top of the pay band and work a minimum of 35.5 hours per week.
Main duties of the job- Patient clinical record entries, and patient specific documentation.
- Support the Occupational Therapist and Nurse in charge in completing delegated duties.
- Maintain confidentiality, dignity, privacy and respect for patients at all times.
- Complete nursing and physical observations, and functional assessments of patients.
- Support with the development of therapy goals and person centred care plans.
- Support with the organisation and development of the Activity ward planner.
- Delivering regular rehabilitation activity sessions and groups.
- Supporting with reablement and liaise with community groups.
- Assisting in house keeping, domestic, and food preparation duties.
- Escort patients from the ward to attend appointments and community activities ensuring their safety at all times.

Person SpecificationQualifications
- Level 3 qualification (e.g., A-level, NVQ3) in Care or equivalent qualification or experience in care sector.
- Level 2 qualification (e.g., GCSE Grade A C or Grade 9 4, NVQ2) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post or equivalent qualification or experience in care sector.
- Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust.
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
- I understand the Trust will not provide sponsorship to work for this role, and employment on a time limited visa doesn't guarantee sponsorship at point of expiry.
- Experience of working with people, preferably in a caring.
