Key responsibilitiesClinical leadership.
- Lead comprehensive assessments, diagnostics, and treatment planning for patients with complex and diverse health needs.
- Utilise advanced clinical skills to manage high-risk cases and make critical clinical decisions autonomously.
Patient care excellence.
- Develop and implement advanced care plans tailored to individual patient needs, ensuring high-quality outcomes and patient safety.
- Provide expert guidance and support to patients, families, and the healthcare team, ensuring holistic and integrated care.
Innovation and Improvement.
- Drive improvements in clinical practice through evidence-based approaches, contributing to the development and implementation of new protocols and procedures.
- Lead or participate in research and quality improvement initiatives to advance primary care practices.
Team collaboration.
- Work closely with GPs, nurses, and other healthcare professionals, fostering a collaborative approach to patient care and service delivery.
- Participate in multidisciplinary team meetings, case reviews, and strategic planning sessions to enhance service provision.
Education and mentoring.
- Mentor and support junior staff, providing training and guidance to enhance their clinical skills and knowledge.
- Engage in continuous professional development, staying abreast of advancements in clinical practice and healthcare delivery.
Compliance and governance.
- Ensure all clinical activities comply with NHS policies, care quality commission (CQC) standards, and General Data Protection Regulation (GDPR) requirements.
- Maintain accurate and comprehensive patient records, ensuring confidentiality and adherence to regulatory standards.
Advanced clinical practitioner (ACP) qualification or equivalent.
Master's degree in advanced clinical practice or related field.
Registration with an appropriate professional body (e.g., NMC or HCPC).

Strong analytical skills, capable of interpreting complex data and presenting findings effectively.

Significant post-registration experience in a clinical setting, with proven expertise in managing complex cases.

Comprehensive knowledge of NHS policies, clinical guidelines, and legal and ethical frameworks governing healthcare delivery.
Familiarity with public health issues and strategies for managing chronic diseases and high-risk patient populations.
Awareness of quality improvement methodologies and their application in clinical settings.
Understanding of GDPR and its implications for patient care and record-keeping.
