GP Practice Clinical Pharmacist

We are seeking an enthusiastic and motivated Clinical Pharmacist (Independent Prescriber) to join our large, dynamic, and forward-thinking GP practice.
You will play a key role in delivering safe, effective, and evidence-based prescribing, working closely with GPs and the wider multidisciplinary team to improve patient outcomes.
This is an excellent opportunity for a pharmacist who is passionate about patient-centred care and is looking to develop their clinical role within primary care.
Main duties of the job- Undertake clinical medication reviews for patients with complex needs, including those with long-term conditions and polypharmacy
- Optimise prescribing to improve patient outcomes and reduce risk
- Manage repeat prescribing systems and undertake medicines reconciliation, including post-discharge reviews
- Monitor patient responses to medication and adjust treatment plans accordingly
- Support the implementation of NICE guidance and local prescribing policies
- Contribute to QOF, clinical audits, and medicines safety initiatives
- Provide prescribing advice and support to GPs, nurses, and the wider practice team
- Respond to MHRA alerts and ensure implementation of safety guidance
- Ensure compliance with CQC standards and clinical governance requirements
- Work collaboratively with PCN pharmacists and pharmacy technicians

Person Specification Qualifications- - Registered Pharmacist with GPhC
- - Experience working in primary care or general practice
- - Strong clinical knowledge and commitment to evidence-based practice
- - Excellent communication and interpersonal skills
- - Ability to work independently and as part of a multidisciplinary team
- - Experience with EMIS
- - Previous involvement in QOF and clinical audit
- - Experience managing long-term conditions
