Job Description
Responsibilities
  • Treat patients who are ill or believe that they might be ill
  • Perform telephone consultations, telephone triage and online consultation as needed
  • Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Perform home visits as agreed
  • Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
  • Conform to practice procedures for QOF, prescribing incentive schemes, PBC and other enhanced services and directives as stipulated by the National Bodies/ ICB or Tudor Surgery
  • Complete referral letters as necessary for patients seen
  • Review and sign repeat prescriptions as and when required
  • Clinical Governance/Quality Assurance: Follow and contribute to protocols to achieve a high score on the Quality and Outcomes Framework
  • Enhance current polices and procedures within the practice as required
  • Ensure data is entered in an efficient and accurate manner on to the practice computer system
  • Promote confidentiality (in line with Caldicott principles) within the practice
  • Deal with non-NHS correspondence (such as life insurance, holiday cancellations and work related letters)
  • Deal with pathology results communications from Out of Hours Service
  • Ensure that repairs are performed to a high standard and in accordance with Health & Safety guidelines
  • Jobs are provided by the Find a Job Service from the Department for Work and Pensions (DWP).
