Job Description
Responsibilities 
- Treat patients who are ill or believe that they might be ill
- Perform telephone consultations, telephone triage and online consultation as needed
- Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Perform home visits as agreed
- Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
- Conform to practice procedures for QOF, prescribing incentive schemes, PBC and other enhanced services and directives as stipulated by the National Bodies/ ICB or Tudor Surgery
- Complete referral letters as necessary for patients seen
- Review and sign repeat prescriptions as and when required
- Clinical Governance/Quality Assurance: Follow and contribute to protocols to achieve a high score on the Quality and Outcomes Framework
- Enhance current polices and procedures within the practice as required
- Ensure data is entered in an efficient and accurate manner on to the practice computer system
- Promote confidentiality (in line with Caldicott principles) within the practice
- Deal with non-NHS correspondence (such as life insurance, holiday cancellations and work related letters)
- Deal with pathology results communications from Out of Hours Service
- Ensure that repairs are performed to a high standard and in accordance with Health & Safety guidelines
- Jobs are provided by the Find a Job Service from the Department for Work and Pensions (DWP).
