Job Description

You will work on a rota basis between 7.45am and 6.30pm Monday to Friday. The post is 25 hours per week. You will be responsible for reception duties and general support to the multidisciplinary team.

You act as the central point of contact for patients, distributing information, messages and inquiries to the clinical team and liaising with multi disciplinary team members and external agencies.

About us

We are looking to add a highly motivated, patient focused, hardworking receptionist (Care Navigator) to join our "Modern General Practice" team.

Having recently moved to the "Modern General Practice" appointment model, you will assist patients to navigate the new system, which involves an online request form and the duty GP allocating appointments according to medical needs.

We are a medium sized family practice with around 6,500 patients in the Clayton le Moors area and offer training opportunities for future doctors, GPs and nurses.

Main duties of the job
  • Maintain and monitor the practice appointment system
  • Process personal, telephone and e requests for appointments
  • Answer incoming phone calls, transferring calls or dealing with callers' requests appropriately, recording the contact in the patient record
  • Signpost patients to the correct service where appropriate
  • Initiate contact with and respond to requests from patients, team members and external agencies
  • Clinically code data on EMIS
  • Photocopy documentation as required
  • Ensure new and temporary registrations and relevant patient information are passed to the registrations administrator as required
  • Input data into patient healthcare records as necessary
  • Direct requests for information, i.e., SAR, insurance/solicitor letters and DVLA forms to the administrative team
  • Manage all queries as necessary in an efficient manner
  • Carry out system searches as requested
  • Maintain a clean, tidy, effective working area at all times
  • Monitor and maintain the reception area and notice boards
  • Support all clinical staff with general tasks as requested
Additional responsibilities
  • Partake in audit as directed by the management team
  • Action incoming and outgoing emails and correspondence as necessary
  • Scan patient related documentation and attach scanned documents to patient health records
  • Complete opening and closing procedures in accordance with the duty rota
  • Support the Prescription Administrator in the management of repeat prescriptions
  • Participate in the receptionist rota to maintain appropriate cover
  • Support the Reception Supervisor in producing and maintaining the duty rota
  • Support the practice manager with all Patient Participation Group related matters
  • Support the health promotion lead and display promotional material on the allocated notice boards and in the waiting room
Qualifications
  • Educated to GCSE level or equivalent
  • GCSE Maths and English (Grade C or above - or equivalent)
  • NVQ Level 2 in Health and Social Care

  • Working with the general public

  • DBS check essential
