Job Description
General Medical Practitioner (Primary Care Network)

We are looking for GPs to join our dynamic PCN multidisciplinaryteam to cover 9-10 sessions. Part time welcome. (Please include in your application how many sessions you can do).

They will join ourcurrent PCN employed GP to make a team of 3 PCN GPs. This growing GP team willcontribute to our team of teams approach that connects the 7 practice teamswith our PCN pharmacy, mental health, practice support, and social prescribingteams.

The role will involve some sessions of practice-based GPclinics. There will be access to senior GPsfor advice and support, and opportunity for supervision and debrief as needed.

The remaining sessions will be used to develop a PCNbased portfolio career, pursuing interests that span population health andproactive personalised care. We have a number of PCN projects where a GP canslot in to contribute as part of our innovative approach to metabolic health,chronic pain, mental health or women's health. However, we also remain open toideas that new GPs bring, and consider other network level projects they wouldlike to develop

Our approach as a PCN is to work closely in partnership withour local population, todevelop a collaborative community of support. This incorporates the newdisciplines of social prescribing, health coaching, care co-ordinationalongside a wide primary care MDT (including mental health workers, pharmacistsand pharmacy assistants, first contact physio, GP assistants and digital lead)

Main duties of the job

We anticipate this role will include (by mutual negotiation):

-4-6 sessions / week

-Good supervision and mentorship with senior PCN GPs and managers

-Opportunity to join innovate approaches to Metabolic health and Chronic pain, including group consultation, lifestyle interventions, peer support.

-Working as part of our PCN MDTs

-Opportunity to develop leadership skills (work alongside PCN Clinical Director)

-Time for personal development, and non- patient facing learning

-Mix of core General Practice clinics in practices and more innovative services (as above)

We have well embedded teams that widen our multidisciplinary (MDT) and multi-professional approach. These teams cover mental health, social prescribing, pharmacy and general practice support (digital lead, and GP assistants).

Job responsibilities

In accordance with the agreed PCN timetable, the post-holder will undertake a variety of duties, including:

Conducting surgery and telephone consultations, addressing patient queries, and visiting patients at home.

Checking, signing, and authorising prescriptions, while managing queries, paperwork, and correspondence in a timely manner.

Making professional, autonomous decisions regarding presenting problems, whether self-referred or referred by other health care workers.

Assessing the healthcare needs of patients with undifferentiated and undiagnosed issues.

Screening patients for disease risk factors and early signs of illness.

Developing care plans in consultation with patients, in line with current disease management protocols.

Providing counselling and health education.

Admitting or discharging patients from the caseload and referring them to other care providers as appropriate.

Recording clear and contemporaneous consultation notes to agreed standards.

Participation in education/teaching of wider MDT teams including medical and non medical teams

Compiling and issuing computer-generated acute and repeat prescriptions, avoiding hand-written prescriptions whenever possible.

Prescribing in accordance with the Practice prescribing formulary or generically, as clinically appropriate.

Undertaking emergency patient visits during surgery hours, if clinically indicated.

Participating in on-call sessions and home visiting sessions as allocated in the practice rota.

Committing to lifelong learning and audit to ensure evidence-based best practices.

Contributing to evaluation, audit, and clinical standard setting within the organisation.

Assisting in the development of computer-based patient records.

Person Specification Qualifications
  • Newly Qualified General Practitioner (with under 2 years from qualification) Registered General Practitioner (GMC) Current UK/EU Driving License Registered on local Performer's List Willingness to complete an Enhanced DBS Check Up-to-date vaccinations Not previously been substantively employed at a GP Practice General understanding of the primary care contract
Experience
  • Experience of working in a primary care environment Experience of continued professional development Experience of QOF and clinical audit
  • Experience of using Systmone/EMIS clinical systems
