Job Description

The post-holderwill manage a caseload and deal with a wide range of health needs in a primarycare setting, ensuring the highest standards of care for all registered andtemporary patients.

Main duties of the job

To undertake duties governed by the contract and Direct and LocalEnhanced Services comprising a variety of duties, including face to face surgery consultations,telephone consultations and queries, visiting patients at home, checking andsigning repeat prescriptions and dealing with queries, paperwork andcorrespondence in a timely fashion.

Job responsibilities

ClinicalResponsibilities

To undertakeduties governed by the contract and Direct and Local Enhanced Servicesincluding:

A varietyof duties including face to face surgery consultations, telephone consultationsand queries, visiting patients at home, checking and signing repeatprescriptions and dealing with queries, paperwork and correspondence in atimely fashion

Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation

Assessingthe health care needs of patients with undifferentiated and undiagnosedproblems

Screeningpatients for disease risk factors and early signs of illness

Inconsultation with patients and in line with current practice disease managementprotocols, developing care plans for health

Providingcounselling and health education

Admittingor discharging patients to and from the caseload and referring to other careproviders as appropriate

Recordingclear and contemporaneous consultation notes to agreed standards

Collectingdata for audit purposes

Administration- dealing with queries, patient test results, referral letters, task, emailsand other paperwork and correspondence in a timely manner

Compilingand issuing computer-generated acute and repeat prescriptions

Prescribingin accordance with the practice prescribing formulary (or generically) wheneverthis is clinically appropriate

In generalthe post-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.

Cover allthe clinical management of the patients in surgery as appropriate includinginitiating investigations, reviewing results, making referrals to secondarycare or to other providers as appropriate

Developcare and treatment plans in consultation with patients and in line with currentPractice disease management protocols as well as wider guidance such as thatfrom NICE.

Person Specification Qualifications
  • Full GMC registration.
  • Completion of Certificate of Training (CCT) in GP dated within the last 2 years.
  • Performers List registered
  • The General Medical Practitioner must not be:
  • a. Prohibited from performing any such service by regulation 24 of the National Health Service (Performers Lists) (England) Regulations 2013;
  • b. Suspended from the list of medical practitioners maintained and published by NHS England in accordance with section 91 of the NHS Act 2006 or from the registers kept under section 2 of the Medical Act 1983;
  • c. and/or Subject to interim suspension under 41A of the Medical Act 1983.
  • Experience of working in NHS General Practice.
  • Keeping up with CPD and good knowledge of current guidelines.
  • Understanding of QoF, Medicines Management and the general working of GP practices.
  • Understanding of EMIS Systems.
  • Excellent communicator and team player.
