This is a 3 year Fixed term role 35 hours per week, rota based across 7 days including mornings, afternoons, evenings and weekends.
Scope - To be a member of a multi-disciplined team usingholistic approachesto support Depaul service users. Working toa housing-led, low threshold, harm reduction approach,where serviceusers receive a high degree of support and advice.Depaul has a commitment to service user participation andto empoweringour service users to manage their own lives.
The aim of the role is to support and signpost service usersinto servicesthat will improve their housing, health andwellbeing whileimproving the serviceuser'squality of life. This isachieved throughthe provision of engagement and individualisedsupport plans, targeted interventions,signpostingand group work.You willwork proactively form strong relationships and gainservice users' trust, striving toprovide the best possible service levelsat alltimes.
Key Responsibilities Service user focus- To provide service user support in a person lead manner, whichis both professional and respectful.
- Assess the safety and wellbeing needs of the service user anddevelop plans which address and reduce identified risks.
- In partnership with service users, to assess their need sand developsupport plans, which are flexible and respond tothe changingneeds of the service users.
- Supporting people to engage with services, to stabilize and settle into accommodation settings
- To build effective, appropriateand supportiverelationships withservice users, developing support plans with the aimof reducingre-offending
- Be responsible and resilient in the face of challenging behaviorand individuals who may be in crisis;discloseand discussrisks responsibly,operatewithin existing datasharing frameworks; acknowledge areas of limitation; and liaisewith alllinked professionals responsible.
- Developing and implementing Resettlement Plans and reviewingprogress with Service Users.
Facilitate individuals toidentifyandmaintainappropriate moveon accommodation providing practical helpand supportthroughout this transition.
- To work with service users and relevant external agencies in orderto deliver meaningful and effective key workingTo supportservice users to integrate with their communitiesin line with good neighborhood principals.
- To advocate on behalf of service users to achieve the best outcomesfor them.
- Provide outreach work within the community based on servicedelivery needs
- To ensure that Health and Safety standards are met within theservicein accordance withDepaul's Health andSafety policyand procedures.
- To undertake assigned duties regardingthe overallhealth andsafety and security of the service andutilizeavailable securitysystems to manage risk relevant to the service.
- To ensure completion of all relevant wellbeing and health andsafety checks
- Carry out service administration tasks such;upkeep of clientrecords, financial transactions, internal andexternal reports, incident and accident management and otherrecord keepingrelevant to the roles.
- To remain vigilant to IT and cyber risks and comply at alltimeswith Depaul's IT Security policies.
- To be a contributing team player, taking part in handovers, teammeetings, core training, the mentoring ofVolunteers andsupporting other team members in ensuring alldecision makingisappropriateand consistent
- To at all timesundertake your role in a professionalmanner maintainingahigh qualitystandard of work in linewith DepaulValues and ethos.

Additional Requirements- Ability to work on a rota basis (7-dayweek -Mornings, Evenings& Weekends)
- Essential Criteria for community outreach Case Workers: Access to a car insured for business purposes during workinghours, and a valid driving licence.
