Frailty Nurse Practitioner - Advanced Clinical Practice

NHS is seeking an experienced Frailty Specialist Nurse/Practitioner to join the Integrated Neighbourhood Team in Truro. This role involves working closely with the wider multidisciplinary team to provide a pro-active and responsive service for patients living with frailty.

The successful candidate will be responsible for assessing individual patient needs and initiating appropriate evidence-based treatment. Significant experience in primary care or elderly care is essential.
