Frailty ACP Lead - 24/7 Community & Acute Care

Buckinghamshire Healthcare NHS Trust is seeking an Advanced Clinical Practitioner to lead the expanding ACP team and provide autonomous care within frailty pathways. You will supervise, plan services and ensure high standards of practice across Stoke Mandeville, Amersham and Buckingham sites.

You will rotate across acute and community settings, support with education, clinical governance and 24/7 coverage of frailty services, contributing to a patient-centred strategy and NHS improvements.
