Location: Hammersmith, London (Hammersmith & Fulham Council)
Contract: Initial 3 months
Hours: 36 hours per week
Pay: £27 per hour (via umbrella company) (£21 PAYE an hour)

Are you an experienced Early Help or Family Support Worker with a strong background in delivering early intervention services? Hammersmith & Fulham Council is seeking a passionate and dedicated Family Support Practitioner to join their multi-agency team in Hammersmith.
Key Responsibilities:
- Collaborate with professionals from CAMHS, schools, school nursing teams, police, health services, and the voluntary sector to deliver coordinated and consistent support to vulnerable families.
- Deliver 1:1 support plans, establish baselines, agree outcomes with families, and review progress regularly to ensure positive and sustained change.
- Ensure the voices of children, young people, and parents/guardians are consistently sought, heard, and appropriately represented.
- Record feedback, track family outcomes, and provide evidence of impact to line managers.
- Maintain timely and accurate case notes and service plans on local authority systems, in line with information governance policies.
- Proven experience as an Early Help or Family Support Worker, with a track record of managing and progressing a caseload independently.
- Demonstrated experience completing Early Help Assessments and facilitating Team Around the Family (TAF) meetings.
