District Nurse Sister / Team Leader

Weare looking for an enthusiastic band 6 District Nurse team leaders to delivercare to the housebound population within the primary care setting. You will becaring for patients with long term conditions and complex health needs,developing your skills and knowledge, whilst working towards an integratedhealthcare structure. You should hold the Specialist Practitioner Degreequalification (SPQ). You will be responsible for leading teams and liaisingwith stakeholders ensuring the delivery of safe effective care. You should beable to demonstrate you can make clinical decisions to provide evidence-basedpractice and care planning in response to clinical assessments.

Main duties of the job
  • The successful candidate will perform holistic assessments of patient nursingneeds. Plan, assess, implement and evaluate care delivery according to changinghealth needs utilising prescribing skills.
  • Supportthe care management of complex/continuing care packages.
  • Ensurepractice is evidence based through undertaking audit and critically appraisingliterature to achieve a high standard of care.
  • Toestablish and maintain effective communication with stake holders, individualgroups and communities in complex and potentially challenging situations.
  • Involvepatients and carers in the planning and delivery of care.
  • Toocompetently perform a range of clinical skills related to the post.
  • Maintainaccurate records in accordance with NMC Standards.
  • Toparticipate in the health needs assessment of the practice population toidentify areas of unmet need.
  • Abilityto work without direct supervision and to solve problems.
  • Undertakechronic disease management reviews of the housebound patient using appropriatetemplate
  • Workon a rota basis to facilitate weekend working across the district nursing teams

Job responsibilities

You should be able todemonstrate excellent Leadership Skills. Maintain effective communication.Promote lifelong learning amongst staff.

The successful candidate willperform holistic assessments of patient nursing needs. Plan, assess, implementand evaluate care delivery according to changing health needs utilisingprescribing skills.

Delegate the workload toappropriate team members.

Day to day responsibility forthe designated caseload.

Support the organisationspublic health agenda.

Identify any limitations/gapsin professional or clinical knowledge.

Facilitate the placement of pre and postregistration nursing students.

Support the development of thenursing team by teaching and assessing other member of the team includingnursing auxiliaries.

Teaching of patients/carersand other professionals to support patients to maintain their optimum level ofindependence.

Person Specification Qualifications
  • Evidence of GCSE Mathematics and English Language (grade A -C / 9-4), or equivalent qualification including functional skills Level 2
  • Willingness to undertake District Nursing (NMC) Specialist Practitioner Apprenticeship
  • Teaching qualification
