The post holder will assist in the provision of an efficient, effective and comprehensive administrative service supporting the Integrated Discharge Team and Enhanced Care Team. The post holder will be expected to work alongside ward staff/Therapies/Patient flow team as well as external partners to facilitate discharge flow and with the organisation of Enhanced Care services. And support achievement of key performance indicators.
Using appropriate technologies and equipment the post holder will produce all work outputs to a high standard meeting the organisations requirements for Patient Safety, Information Governance and CQC recommendations in adherence with Trust policies, procedure and guidelines.
Main duties of the jobDischarge Process Facilitator Band 3
We are excited to offer an opportunity within Patient Flow and the Integrated Discharge Team based at UNHT. You will work closely with the ward staff as well as local partners including social care and voluntary agencies to support care transitions.
We require a person who has excellent communication skills and who is confident working across professional boundaries. You will need to be flexible, adaptable and a good team player with the ability to work as part of a team as well as independently provide a whole person approach to the care and management for patients within this setting.
This is an exciting and opportunity to work with Patient Flow and Integrated Discharge Team to support the administration and flow of patients through the system including scheduling transport.

Job responsibilitiesKey Result Areas
- Keep Discharge flow maintained and support update of Patient Discharge Optimisation Tool , OPTICA
- Schedule transport for discharge for all patients across the trust.
- Access patient details using the trust electronic systems/Trakcare and SystmOne.
- Inputting information onto active clinical notes.
- Supporting the wards with paperwork required to discharge a patient
- Assist in the process of patient flow through to ensure compliance with the Trust KPIs
- Build strong communication skills with all members of the Multi Disciplinary Team ensuring that the most up to date patient information is available
- Address discrimination and harassment, and promote equality and diversity in the workplace
- Reduce sickness absence work place accidents and promote zero tolerance on violence against staff
- Establish good working relationships throughout the hospital and with partners in the Local Authorities.
- Deal with queries from service users including patients and their representatives in a manner that is consistent with legislation, policies and procedures whilst demonstrating understanding, empathy and discretion, taking accurate written messages and passing more complex queries to relevant staff members where appropriate.
- Assist in planning and prioritising of workload to ensure that all service requirements and priorities are met
- Data entry and retrieval of results/information where appropriate through the effective use of available technologies
- Under the direction of the Senior Clinical Professional/Site Managers, assist in providing interim short-term cover throughout the work area where appropriate to cover sickness absence/annual leave where appropriate
- Assist in ensuring a healthy, safe and secure working environment for self and work team
- Any other ad-hoc duties as and when required in line with service needs and within scope of capabilities
- Demonstrate commitment to IWL principles and flexible working patterns, to meet the needs of the service and staff
- Take responsibility for own education/training
- Supporting enhanced care caseload when applicable
- Demonstrate a good working knowledge of windows based computer systems and be capable of using most Microsoft Office applications, particularly Excel, MS Word and Outlook.
- Experience of dealing with members of the public.
- Trac care knowledge and understanding.
- Ability to contribute to the delivery of an effective and efficient administrative service
- Demonstrate ability to work under pressure/meet tight deadlines whilst delivering high quality work.
- Demonstrate excellent communication and organisation skills
- Experience of working within a healthcare environment
- Experience of working within a multi-disciplinary team
