Discharge & Community Care Officer - Hospital Support

Hertfordshire County Council is recruiting for roles within the Integrated Discharge Team at Watford General Hospital. You will respond to individuals in need of social care support, promoting safe discharges and person-centered planning in line with the Care Act 2014 and Mental Capacity Act 2005.

The successful candidate will have a full UK driving licence, experience in social care, and strong communication skills in English.
