Job Description

An exciting opportunity has arisen for a Complex Health Needs Manager to join our Complex Commissioning and Joint Funding Team at Hampshire County Council.

The Joint Funding Team manage all joint funding arrangements between Children's social care and the Hampshire IOW Integrated Care Board.

The Role:

  • Work closely with social care practitioners, and other social care partners to ensure effective identification and support pathways for children with complex health needs.
    Facilitate Multi-Disciplinary Team (MDT) meetings and promote multi-agency collaboration, particularly during periods of crisis or challenge.
  • Negotiate funding agreements with the Integrated Care Board (ICB) to ensure appropriate and sustainable care packages.
  • Serve as the initial escalation point for crisis management of joint funded children.
  • Representing the team and wider department at strategic workshops and meetings.
  • Contribute to the development and refinement of wider team policies and processes to ensure alignment with best practice and statutory requirements.
  • Produce, analyse, and distribute reports to inform service planning and performance monitoring for the joint funding team.

What you'll do:

We are seeking a dynamic leader to coordinate and oversee services for individuals with continuing care needs, complex life needs, and those eligible for Section 117 aftercare. This pivotal role ensures effective multi-agency collaboration, proactive crisis management, and robust funding arrangements to deliver high-quality, person-centred care.

What we're looking for:

  • Proven experience in commissioning or managing services for individuals with complex needs.
  • Understanding of relevant Health and Social Care legislation.
  • Excellent negotiation and stakeholder engagement skills.
  • Experience in multi-agency work and crisis management.
  • Ability to produce and interpret performance data and reports.
  • Strong leadership and facilitation skills.
  • Ability to write clear and conscious information
  • Proactive and able to work autonomously.
  • Proficient in the full Microsoft office suite.
