Job Overview 
Complex Care Nurse (office based)
Location: Dartford
Salary: £35,000 - £39,000 per annum
Hours: Bank Nurse / 0 hours. Monday to Friday availability with some flexibility in unsocial hours to cover branch needs. Office based with occasional travel to clients.

- Accountable for and lead on clinical components of care delivery.
- Support in assessment, mobilisation, and implementation of complex care for adults and children.
- Regularly risk assess individual client needs.
- Provide evidence-based clinical expertise and advice across the pathway and evaluate outcomes.
- Attend meetings and liaise with MDT, commissioners, case managers, and clients to build and manage clinical caseload.
- Act as a role model and leader for staff.
- Facilitate and deliver continuing programme of clinical knowledge/skills training and competency assessments to non-registered staff.
- Delegate clinical healthcare tasks required for clients to maintain quality of life within the community.
- Maintain high standards and proactively manage quality improvement, clinical risks, safety concerns and complaints linked to client caseload.
- Drive best practice to ensure high-quality care.
- Promote compliance with internal policies and standards.
- Keep up to date with national standards related to clinical services.
- Assist branch in identifying best practice regarding client complexity to ensure safe and high-quality care.
- Commit to improving client/family experience at every step of the care pathway.
- Identify, develop and maintain new business streams to support growth of clinical services.
- Registered Nurse with NMC and active PIN.
- Minimum 2 years clinical nursing experience in community or hospital setting.
- Experience in clinical risk management and individualised, person-centred care planning.
- Experience safeguarding adults and children.
- Effective verbal and written communication skills.
- Ability to work independently and as part of a team; self-motivated.
- Effective people management skills.
- Negotiation, influence, escalation, and advice-seeking ability.
- Experience interpreting and analysing data and meeting deadlines.
- Excellent IT and presentation skills.
- Knowledge of clinical governance principles.
- Ability to work autonomously and proactively.
- Can-do approach.
- Valid UK driving licence; access to car desirable (alternative travel arrangements possible).
- Demonstrated leadership skills.
