Complex Care Mobilisation Lead - Build Home-Based Packages

Alternative Futures Group in the North West is seeking a Mobilisation Specialist to launch new complex care packages. You will work closely with individuals with complex health needs, their families and healthcare professionals to build personalised support that enables independent living at home.

With at least 2 years of clinical care experience and NVQ/QCF Level 2 in Care, you'll travel across services and shadow-train new teams, delivering hands-on care in areas like medication management,
