Community Nurse

Working with your community nursing team, you'll help deliver safe and person-centred care that supports people to stay well at home. You'll be part of an integrated service that provides high-quality nursing care to adults with a wide range of needs. Your work will help people manage their health, stay independent and feel supported in their own homes and residential settings. You will join a friendly and committed team who work closely with colleagues across the Integrated Nursing and Conditions services. • Carry out holistic assessments and develop clear, person-centred care plans

• Work with colleagues and partners to support joined-up community care
• Use and update the Electronic Patient Record (EPR) to maintain accurate notes
• Support people, families and carers to understand and manage health needs

Person Specification Qualifications Essential

• Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration

• Demonstrate an understanding of the role of a community nurse
• Knowledge of legislation impacting on service delivery
• Knowledge of the modernisation agenda
• Demonstrate an understanding of evidence based practice

• Demonstrate an understanding of the MDT
• Basic IT skills
• Knowledge and skills in the assessment process

• Able to mentor students
