Community Mental Health Practitioner

The purpose of the Rapid Transfer Dementia Service is to respond to people who have dementia, symptoms of dementia, cognitive impairment or delirium who are experiencing and escalation of those symptoms or a degree of crisis, and who may require physical, mental or social healthintervention.
The service will deliver specialist assessment, treatment and intensive support to support the transition between care and nursing homes.

The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless youhave a disability as defined by the Equality Act 2010).
Main duties of the job- Ability to work flexibly and creatively, planning and problem solving to meet the individual needs
- Ability to communicate with sensitivity, tact and persuasion and able to communicate where there are barriers to understanding.
- To make judgements involving a range of facts, in situations which require analysis or comparison of a range of options.
- Ability to work alone using initiative and co-operation as part of a team
- Professional credibility.
- Knowledge of dementia, cognitive impairment, the impact of diagnosis and treatment options.
- Knowledge of the Mental Health Act 1983 and the Mental Capacity Act 2005.
- Minimum of 2 years post registration experience within a Community or Inpatient Service for Older People with Mental HealthNeeds.
- Experience of working with people living with dementia/cognitive impairment.
- Experience of working liaising with a variety of stakeholders involved in clinical care.
- To be able to assess the care needs of the Service Users and carers.

Job responsibilitiesTo be able to plan, deliver and review patient care using To be able to offer clinical advice to Service Users and carers. To undertake risk assessments in relation to vulnerable service users providing detailed advice regarding risk managementplans. The need to discuss care issues that service users, carers or family members may find distressing

Person Specification band 6 post- 2 years experience as a qualified practitioner
- experience working with people living with dementia
- having a full driving licence and able to drive round East Kent
- Excellent knowledge of care planning, risk assessment
- having confidence to work with families through triangle of care
- To have completed practice assessors course
- To contribute to quality improvement
- Being able to work independently and complete reviews of patients
- 2 years experience as a qualified practitioner
- Experience working with patients living with dementia, with complex needs
- Understanding of triangle of care
- have a full driving licence and able to drive round East Kent
- Excellent understanding of risk
- Being able to develop care plans for complex patients on the case load
- have a practice assessors course to have students
- being able to report incidents effectively
- To contribute to quality improvement
