Dorset HealthCare University NHS Foundation Trust - Community Mental Health Practitioner. The closing date is 19 February 2026.
Main duties of the job- To be personally responsible and professionally accountable for working with people in the community with severe mental illness who have multiple and complex needs.
- To provide high quality care and treatment in conjunction with other professionals through the Trust's agreed multi-disciplinary assessment and treatment/ care programmes.
- To participate as an active member of the Home Treatment Team and take a lead role in providing high quality and innovative services to people with severe mental illness.

Person Specification Knowledge, skills and training- Registered Mental Health Nurse / Social Worker / Occupational Therapist
- Experience as a Practitioner working within a Community Mental Health Team
- Experience of receiving clinical supervision
- Ability to work and communicate across multi agencies
- Responsibility for delivery of care
- Ability to delegate appropriately
- Positive attitude to team and multi agency working
- Practice supervisor/assessor
- Basic IT skills, normally obtained through practice or practical training
