Overview

Dorset HealthCare University NHS Foundation Trust - Community Mental Health Practitioner. The closing date is 19 February 2026.

Main duties of the job
  • To be personally responsible and professionally accountable for working with people in the community with severe mental illness who have multiple and complex needs.
  • To provide high quality care and treatment in conjunction with other professionals through the Trust's agreed multi-disciplinary assessment and treatment/ care programmes.
  • To participate as an active member of the Home Treatment Team and take a lead role in providing high quality and innovative services to people with severe mental illness.

Person Specification Knowledge, skills and training
  • Registered Mental Health Nurse / Social Worker / Occupational Therapist
Job Specific Experience
  • Experience as a Practitioner working within a Community Mental Health Team
  • Experience of receiving clinical supervision
  • Ability to work and communicate across multi agencies
  • Responsibility for delivery of care
  • Ability to delegate appropriately
Managerial/Supervisory experience
  • Positive attitude to team and multi agency working
  • Practice supervisor/assessor
  • Basic IT skills, normally obtained through practice or practical training
