Community Mental Health Practitioner

An exciting opportunity has arisen for a Band 5 nurse to join our innovative South Locality Team. The team offers a range of interventions which are person centred and fully collaborative with the Patient.

The post holder will work as a member of a multidisciplinary CMHT providing a service to clients experiencing mental health difficulties of a serious and enduring nature. You must demonstrate a flexible approach centring around the needs of patients, their families and significant others.

You will develop and maintain close and professional working relationships with colleagues in the team and across other statutory and voluntary agencies.

We are looking for an enthusiastic person with excellent team working skills, who is keen to develop their knowledge and skills in this area. You should be a competent and motivated practitioner with appropriate demonstrable experience. You will work as part of a multi-disciplinary team providing assessment and therapeutic intervention in the community.

Main duties of the job

To carry out nursing interventions within the community as required.

To assist in the development of the Nursing Team, working with other agencies to ensure the highest standard of patient care.

PRINCIPAL DUTIES / MAIN TASKS.

Managing a team caseload under supervision in accordance with sound nursing practice and principles.

Undertaking regular supervision and PDR in accordance with Trust policy.

Promoting and Maintaining the highest standards and attitudes towards the care of patients / clients. Assisting colleagues in maintaining an effective and efficient service.

Assisting, developing, implementing and evaluating plans of care, ensuring therapeutic and recreational programmes for patients / clients individually and in groups, in the Community.

Ordering, storage, monitoring and administration of medication.

Liaison within the multi-disciplinary care team, including carers, voluntary agencies etc.

Person Specification Qualifications
  • Current registration as a level one Mental Health nurse/RMN
  • Possession of post graduate Qualification in relevant field
Experience
  • Working with people experiencing complex mental health problems
  • Assessing mental health needs
  • Managing and planning care and risk management
Knowledge
  • Knowledge of CPA, Mental Health Legislation
  • NICE Guidance
  • Recovery focussed interventions
  • Use of information systems and I.T literate
  • Knowledge of dialog and dialog+
Skills
  • Excellent communication skills Both written and verbal
  • Good organisation skills, time management
  • Team player
  • Committed to engagement and collaboration with carers and service users in care planning and service development
  • Understanding of Physical health observations, monitoring and interventions
  • Venepuncture or willingness to undertake training and utilise on completion
  • ECG or willingness to undertake training and utilise on completion.
