Community Link Worker - Social Prescribing & Wellbeing

Citizens Advice Milton Keynes is seeking a Social Prescribing Link Worker to join a local project across three Milton Keynes GP surgeries.

You will take referrals from Ascent PCN practices and external agencies, identify health and wellbeing needs, and co-produce simple, personalised care plans that connect patients to community activities and services. Travel between sites is required and a DBS check will be undertaken.
