Community Connecting (CC), empowers individuals to become valued members of their communities

The role of Community Coordinator assists young people and adults with special educational needs, disabilities, and autism to find out more about their local community and how they can access some of the following; voluntary work, socialising, meeting friends or joining a club, further education, including college courses and other training opportunities, using their community facilities to do things that they enjoy and travel training.

As a Community Coordinator, you'll be the starting point that first step to independence. Working within a highly committed team, you will:

  • Explore communities and potential opportunities for young people and adults with disabilities.
  • Work with the families, volunteer sector, community groups and public travel provision
  • Undertake travel training for confidence, to outcomes and education with the aspiration of independence
  • Provide appropriate coaching support to customers to achieve identified goals, fading from support when the customer is confident
  • Providing vital support to families and networks.

We're seeking a passionate, committed, and self-motivated individual with the essential qualifications and skills, who are comfortable with working beyond the typical 9-5 day, that can include weekends. If you have good communication and organisational skills, experience of supporting people, have a knowledge of learning disability or autism, and the confidence to liaise with community groups and volunteer organisations, then we want to hear from you.

We are currently recruiting for a full time Community Coordinator, and you will be working countywide but your working base will be Monkton Park, Chippenham.

Community Connecting is part of the wider Employment and Community Skills Team that have an established pathway to work for young people with SEND.
