Job Description

Overview

Nottingham West PCN covers 12 practices with a well-established clinical pharmacy team, including a pharmacist lead cardiovascular service for hypertension and lipids across the PCN. The service has won a PresQipp award and was shortlisted for the National General Practice awards. We are a forward-thinking PCN with a large clinical pharmacy team, offering support for newly qualified pharmacists and opportunities for growth for more experienced pharmacists. Medicines safety is central, with PCN-level monitoring of high-risk medicines and innovative system one visualisations for drug monitoring.

You will be a pharmacist who will act within professional boundaries, supporting and working alongside a team of Pharmacists in General Practice and supported by a Neighbourhood Lead. Initially you will be based within two practices, with PCN staff potentially covering different practices or the pharmacy cardiology team depending on service needs.

Main duties of the job

Job responsibilities
  • Neighbourhood team working and management of neighbourhood technicians
    Manage and support neighbourhood PCN technicians in development and delivery of CPPE primary care education pathway and key PCN workstreams, including IIF targets; provide face-to-face support and collaborate with other PCN pharmacists.
  • Patient-facing clinical medication review
    Undertake clinical medication reviews and produce recommendations for Senior Clinical Pharmacist, Nurses and/or GP on prescribing and monitoring. Work with care home staff to improve safety of medicines ordering and administration.
  • Patient-facing domiciliary clinical medication review
    Undertake domiciliary reviews, attend and refer patients to multidisciplinary case conferences.
  • Management of common/minor/self-limiting ailments
    Manage caseload within scope of practice; signpost to community pharmacy and refer to GPs or other healthcare professionals where appropriate.
  • Patient-facing medicines support
    Provide patient-facing clinics for questions, queries and concerns about medicines in the practice.
  • Telephone medicines support
    Provide a telephone helpline for patients with medicines questions and concerns.
  • Medicine information to practice staff and patients
    Answer medicine-related enquiries from GPs, practice staff, other healthcare teams and patients; provide follow-up and monitoring after changes.
  • Unplanned hospital admissions
    Review medicines commonly associated with unplanned admissions and readmissions; conduct audits and implement changes to reduce risk in high-risk groups.
  • Management of medicines at discharge from hospital
    Reconcile medicines after discharge and ensure continuity of supply for high-risk groups.
  • Signposting
    Refer patients to appropriate healthcare professionals for the right level of care in a timely manner.
  • Repeat prescribing
    Develop and implement a practice repeat prescribing policy; review requests and monitor ensuring appropriate monitoring tests are in place; identify high-risk cohorts.
  • Information management
    Analyse medicines data to highlight issues and risks; conduct simple audits and implement changes; respond to alerts and guidance.
  • Implementation of guidelines and formulary
    Monitor prescribing against local formulary and NICE guidance; support formulation of a practice formulary and communicate prescribing messages.
  • Education and training
    Provide education to the primary care team on therapeutics and medicines optimisation.
  • Care Quality Commission
    Work to ensure practice compliance with CQC standards where medicines are involved.
  • Public health
    Support public health campaigns and provide specialist knowledge on public health programmes.
  • Collaborative working relationships
    Recognise roles of colleagues and communicate effectively with stakeholders to support patient care; develop working relationships across locality and with community and hospital pharmacy teams.

Knowledge, skills, and experience

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
  • Minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio
  • Experience and awareness of common acute and long-term conditions in general practice
  • May hold or be working towards an independent prescribing qualification
  • Ability to follow legal, ethical, professional and organisational policies and engage patients in decisions about prescribed medicines
  • NB: qualification level may vary according to the role level

  • Support governance, contribute to workplace vision, engage with Patient Participation Groups, and drive quality improvements within the service
Management
  • Understand national priorities, resource use, and risk management; align with performance management processes and extend service delivery within the team
