This role will suit a clinical Pharmacist looking to build on their clinical knowledge and develop their patient facing skills in supporting the patient journey from the ward to discharge into the community.
The post holder will be supporting inpatient Community Hospital wards at several sites in Dorset, based on their location. They will be ensuring seamless transfer of care back into primary care, supporting the discharge process and identifying patients in need of follow up, providing clinical advice to the staff, patients and our Pharmacy Technicians who provide follow up visits in the patients own homes.
As an independent prescriber, there will be opportunity to prescribe and deprescribe medication on the wards.

Main duties of the jobProvide medicines management support in delivering Pharmacy Services to Wards and neighbourhood health hubs: medicines optimisation and clinical pharmacy service within community hospitals and neighbourhood health hubs promoting a seamless medicines service for transfer into patients' homes. To support and, where necessary, participate in a service which receives patient referrals from external stakeholders for pharmacy patient support in the domiciliary setting that improves patient outcomes.
Person Specification Knowledge, skills and training- Vocational masters degree in Pharmacy (4 years or equivalent) plus one year pre registration training
- Registered with the General Pharmaceutical Council
- Broad clinical pharmacy knowledge
- Good organisational skills
- Postgraduate clinical pharmacy diploma or equivalent experience
- A member of or working towards Faculty membership of the Royal Pharmaceutical Society
- Knowledge of NHS primary and secondary care working
- Demonstrable experience as a practising pharmacist
- Experience of working in a 'clinical capacity'
- Experience of working in a multidisciplinary team
- Able to communicate with patients
- Able to influence other healthcare professionals
- Experience of providing patient information and education
- Experience of coordinating complex activities / programmes of care with other professionals and agencies.
- Effective communicator able to communicate highly complex and sensitive information where there may be barriers to understanding.
- Demonstrates accountability for delivering professional expertise and direct service provision
- Able to work independently.
