Job Description

Salary -Starting at £28,704 (FTE £47,840) dependent on experience

Location -Stratford-upon-Avon (travel required across Stratford practices full UK driving licence and access to a car essential)

Main duties of the job

Stratford PCN is seeking a passionate and motivated Clinical Pharmacist to join our team. This patient-facing role plays a key part in improving health outcomes and supporting the efficiency of the primary care team.

You will provide direct, accessible and timely medicines expertise, including face-to-face consultations. The role focuses on medicines optimisation, improving patient safety, and freeing up GP and clinical time to focus on direct patient care.

Job responsibilities
  • Support patients to get the best from their medicines, including switching medications in line with agreed protocols.
  • Improve repeat prescribing processes within General Practice, promoting repeat dispensing and online ordering.
  • Minimise clinical risk and reduce medicines waste.
  • Work as part of a multidisciplinary team in a patient-facing role, clinically assessing and treating patients.
  • Take direct responsibility for the care and prescribing of defined patient cohorts.
  • Deliver and develop pharmacy-led medicines optimisation services.
  • Ensure practices remain up to date with national guidance and evidence-based practice.
  • Lead and promote medicines optimisation within a specialist area, ensuring best value, reduced harm and improved outcomes.
  • Lead evaluation and audit of service delivery to maintain and improve quality.
  • Work in accordance with Medicines Optimisation principles and up-to-date national guidelines.
Person Specification Qualifications
  • Evidence of post-qualification and continuing professional development.
  • Extensive knowledge of medicines optimisation (Postgraduate Diploma or equivalent experience/training).
  • Member of the GPhC.
  • Understanding of national policies impacting medicines optimisation services.
  • Sound knowledge of medicines legislation and the development of clinical pharmacists within primary care.
  • Experience in a patient-focused role with strong consultation skills.
  • Understanding of legal, governance and ethical frameworks within primary and secondary care.
  • Commitment to promoting equality of opportunity and positive working relationships.
  • Good understanding of primary care.
