We have an exciting opportunity for a Clinical Pharmacist to join theNorth West Leicestershire GP Federation to support the Primary Care Network, as part of our Clinical PharmacistTeam. Clinical Pharmacists are supported and mentored by our Senior ClinicalPharmacist and Leadership Team, and work as part of a multi-disciplinary teamin a patient-facing environment. You would be supported to develop your roleand to become a non-medical prescriber, if not already. The post holder will be employed byNorth West Leicestershire Federation and this position would be based at one of the GP practices within the PCN.
Main duties of the jobYou will take responsibility for medicines management,clinical medication reviews, improving prescribing processes and patient safetyalerts. The post holder will be the primary support to general practice staffwith regards to drugs monitoring, helping to support the repeat prescriptionsystem and dispensing team. You will work with thewider GP Federation Pharmacy team and will be joining a dynamic, forward-thinking team. This is an opportunity to developyourself, and successful candidates will be required and supported to enrol onto(if not already completed) CPPE Primary Care Pharmacy education pathway and, following this, an Independent Prescribing qualification.

Responsible to: This role is directly employed by the North WestLeicestershire GP Federation and the post holder will work for the North WestLeicestershire PCN.
Job Summary
Thepost holder is a pharmacist, who acts within their professional boundaries,supporting and working alongside a team of pharmacists in general practice. Inthis role they will be supported by Lead and Senior clinical pharmacists whowill develop, manage and mentor them.
Thepost holder will work as part of a multi-disciplinary team in a patient facingrole. The post holder will take responsibility for areas of chronic diseasemanagement within the practice and undertake clinical medication reviews toproactively manage patients with complex polypharmacy.
Thepost holder will provide primary support to general practice staff with regardsto prescription and medication queries. They will help support the repeatprescriptions system, and medicines reconciliation on transfer of care andsystems for safer prescribing, providing expertise in clinical medicines advicewhile addressing both public and social care needs of patients in the GPpractice(s).
Thepost holder will provide clinical leadership on medicines optimisation andquality improvement and manage some aspects of the quality and outcomesframework and enhanced services.
Thepost holder will ensure that the practice integrates with community andhospital pharmacy to help utilise skill mix, improve patient outcomes, ensurebetter access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requiresmotivation and passion to deliver excellent service within general practice.
Thepost holder will be supported to develop their role to become a non-medicalprescriber.

Person Specification Experience- An appreciation of the nature of GPs and general practices.
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
- Excellent interpersonal, influencing and negotiating skills.
- Excellent written and verbal communication skills.
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Able to obtain and analyse complex technical information
- Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
- Able to work under pressure and to meet deadlines.
- Gain acceptance for recommendations and influence/motivate/ persuade the audience to comply with the recommendations/ agreed course of action where there may be significant barriers.
- Work effectively independently and as a team member.
- Demonstrates accountability for delivering professional expertise and direct service provision.
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients).

- Up to date basic life support training
- Full Driving Licence
- Mandatory registration with General Pharmaceutical Council (GPhC) which includes revalidation.
- Membership of the Royal Pharmaceutical Society.
- A member of, or working towards, Faculty membership of the Royal Pharmaceutical Society.
- Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
- Independent prescriber or working towards/intent on gaining an independent prescribing qualification.
- Ideally completed PCPEP.
