Location: Calne and Sutton Benger, Wiltshire (across our three GP surgeries)
Salary: Band 7-8a (dependent on experience)
About UsPatford House Partnership is a vibrant group of three GP surgeries serving the Calne community. We are proud to offer a well-established Dispensary and a dedicated in-house Medicines Management Team, comprising three experienced Pharmacy Technicians and a Qualified Dispenser.
As our Primary Care Network continues to strengthen its multidisciplinary model, we are delighted to expand our pharmacy team with a motivated and skilled PCN Clinical Pharmacist under the Additional Roles Reimbursement Scheme (ARRS).
This post is ideal for a pharmacist who enjoys a varied clinical caseload, thrives in a supportive team, and is passionate about safe and proactive medicines management.
Role OverviewThis is a predominantly patient-facing role, working for Patford House Partnership in a PCN role to deliver high-quality medication reviews, long-term condition support, and systems-based medicines optimisation.
You will work closely with our GPs, Nurses, Pharmacy Technicians and Dispensary team to support safer prescribing, streamline processes, and improve patient outcomes across all three sites.
Key Responsibilities-
Patient-Facing Clinical Work
- Undertake medication reviews for all clinically appropriate patients, including Structured Medication Reviews (SMRs).
- Manage polypharmacy, multimorbidity, frailty, high-risk medicines and care-home residents.
- Support long-term condition management.
- Support shared decision-making, deprescribing and patient education.
- Provide input into minor illness presentations, where competent - desirable, not essential.

Medicines Optimisation & Prescribing Support
- Provide clinical support to practice teams with prescription enquiries and medication-related queries.
- Work with our Medicines Management Team to support:
- Repeat prescribing
- Medicines reconciliation
- Safer prescribing systems
- Promote evidence-based, safe and cost-effective prescribing.
- Utilise or work towards Independent Prescribing as part of the role.

Quality Improvement & Leadership
- Provide clinical leadership on medicines optimisation, safety and governance.
- Contribute to DSQS, QOF, PCN DES delivery and enhanced services relating to medicines.
- Undertake audits, QI initiatives and contribute to practice-wide clinical pathways.
