Main duties of the jobWorking within your professional and clinicalboundaries as part of an established multi-disciplinary team to provideexpertise in all areas of medicine related practice in a patient facing role.
Working in partnership with various organisations,to improve the quality and safety of care offered to our patients. Usingindependent prescribing to support better access to medicines, clinicalmedicines management, provide face to face structured medication reviews,tackling polypharmacy and manage long term conditions.
To manage medicines on transfer of care and systemsfor safer prescribing including manage repeat prescriptions, acute prescriptionrequests and antibiotic stewardship, while addressing both the public healthand social care needs of patients across the primary care network practices.
To ensure that PCN practices integrate withcommunity and hospital pharmacy to help utilise skill mix, improve patientoutcomes, ensure better access to healthcare and help manage workload.
This role is pivotal to improving the quality ofcare and operational efficiencies so requires motivation and passion to deliveran excellent service within general practice.

Job responsibilitiesWe are seeking a part-time Clinical Pharmacist (30 hours per week) to join our established team at the Sunderland Central PCN.In this patient-facing role, you will provide expertise in all areas of medicine-related practice while working within your professional and clinical boundaries. You will work in partnership with various organisations to enhance the quality and safety of care offered to our patients. Key responsibilities include:
- Using independent prescribing to support better access to medicines
- Clinical medicines management
- Providing face-to-face structured medication reviews
- Tackling polypharmacy and managing long-term conditions
- Managing medicines on transfer of care
- Managing repeat prescriptions and acute prescription requests
- Supporting antibiotic stewardship
- Addressing both public health and social care needs across PCN practices
- Ensuring integration with community and hospital pharmacy to utilise skill mix and improve patient outcomes
This pivotal role aims to improve the quality of care and operational efficiencies, requiring motivation and passion to deliver an excellent service within general practice.
Person Specification Qualifications- Mandatory registration with General Pharmaceutical Council
- Minimum of two years post qualification experience
- Evidence of continuing professional development
- The post holder must either have completed the CPPE primary care pathway or be working towards it
- UK driving licence
- UK work permit (if required)
- Clinical diploma or other post graduate qualification
- Independent prescriber or working towards qualification
- Minimum of two years post-qualification experience
- Appreciation of General Practice and working alongside General practice staff
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare including clinical audit
