Clinical Pharmacist

We have an opportunity for a Clinical Pharmacist to join their team of staff in the Pharmacy department. This is a full-time role for 37.5 hours a week. Part-time hours will be considered. The role holder will be required to cover a flexible shift pattern with no weekend or on-call working. This is a dual site role with responsibilities split between both The Winterbourne Hospital and The Harbour Hospital.
Responsibilities- Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
- Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
- Support safe prescribing through reconciliation, clinical screening, and dose adjustment (e.g., renal or hepatic impairment).
- Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
- Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.
- A master's degree in pharmacy (MPharm or equivalent).
- GPhC registration as a Pharmacist.
- Previous experience in a clinical hospital pharmacy role.
- Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
- Excellent communication and clinical decision-making skills.
- Ability to work autonomously and as part of a multidisciplinary team.
- Confident using electronic prescribing and medication administration systems (e.g., EPMA).
