Clinical Pharmacist - Primary Care Medicines Optimisation

The NHS is seeking an experienced Clinical Pharmacist to join a forward-thinking primary care network in Tansley, England. You will work in a patient-facing role within a multi-disciplinary team at two GP practices, managing chronic disease and complex medication reviews.
